Sounds like you are maybe dealing with an issue since you're dealing with an ortho. I would discuss this with him or her. Any suggestions from us may cause further harm.

New to the bike. Starting out 20 mins per Ortho Dr. Tension is set a 2. Going forward how do I increase? Speed, time or tension? Help.
