Great question! At the start of each month, I evaluate and set/refine goals as needed. Likewise when milestones are achieved, I set new goals towards achieving the new milestones. About every 3 to 4 months I look at photos, do fitness tests (pushups, squats, flexibility, walking a mile, incline walking, etc) and take measurements to show my progress, as well as highlight any areas I think I'm doing better than I am. Instead of using new years to set resolutions, I tend to go with my birthday. Turning another year old may bring with it changes to the plan. I also set weekly goals of things I'm focused on accomplishing that week. Likewise I set goals for each day. I love planning so goal setting helps a lot! Some times I accomplish a lot, other times I'm behind where I want to be because that goal just takes longer. I'm okay with that. It is worth the effort.
